Seproepidemiological Survey of the Leptospirosis in Korea, 1986-1991

Abstract
Leptospirosis is a zoonotic disease causes by leptospira interrogans. There were many evidences which suggested that leptospirosis had been occured in Korea, but the first human cases confirmed by serological and microbiological methods were
reported
among the farmers and soldiers who had worked on rice-fields after the flood in 1984. Thereafter, outbreaks of human leptospirosis were reported every year. Leptospirosis occurs sporadically and usually among farmers. The incidence of
leptospirosis
peaks at Autumn, especially after the flood.
Leptospirosis is one of the most prevalent infectious diseases which must be controlled urgently, as about 15% of total Korean populations, who are farmers, are exposed to leptospirosis every year and the leptospirosis in Korea is highly
infectious
and
fatal. At preset time, leptospirosis is classified as class ¥± communicable diseases and nation-wide vaccination has been carried out since 1988.
To understand the epidemiological characteristics of leptospirosis in Korea, we collected 14,141 serum specimens from the acute febrile patients who visited our twenty collaborative hospitals scattered all over the country from 1986 to 1991, and
tested
for the presence of antibody to L. inerrogans by microscopic agglutination tests. We analyzed the sexual, age, seasonal and geographic distridution of the disease. @ES The results are summarized as follows.
@EN 1. Among the tested 1,141 cases, 128 cases were seropositive to Leptospira interrogans in 1986, and among the tested 1,773 cases, 219 cases were seropositive in 1987. Seropositive cases were gradually decreased year by year from 1988 and less
than
1% of the tested 4,520 cases was seropositive in 1991.
2. Among the seropositives to leptospira interrogans, male outnumbered female and seropositive rate of male was higher than that of female.
3. More than third quarter of the seropositives were between the age of forties and sixtith.
4. The seropositives were occured most frequently in Chonbuk province and followed by Kyunggi and Chonnam province. Occurrence rate was 37.2% in Chonbuk province and 21% in Kyunggi province and 8.6% in Chonnam province.
5. Among the 573 seropositive cases, 426 cases(74%) were occured between September and November.
